Nasofrontal Angle: How Forehead and Nasal Root Shape the Profile

The nasofrontal angle is the bend where your forehead becomes the nose. You usually trace it through glabella, nasion, and a point or tangent on the nasal dorsum. A smaller angle creates a deeper, sharper root transition. A larger angle creates a flatter, more open transition. It is one of the strongest profile cues you have. It is also method sensitive. Move the forehead tangent or nasion point by a few millimetres and the number moves with it.

How It Is Measured

How is the nasofrontal angle measured?

On a surface profile, mark glabella, the deepest visible point at your root, and a reproducible point or tangent on the nasal dorsum. Draw the forehead segment into nasion and the nasal segment out of nasion. The internal angle at nasion is your nasofrontal angle.

On a lateral cephalogram you can use skeletal landmarks. A photograph sees soft tissue. Those versions answer related questions. Do not share one reference range without adjustment. Forehead curvature also makes a two point line different from a fitted tangent.

Say whether the nasal segment runs toward rhinion, the tip, or a dorsal tangent. A long line to the tip can absorb dorsal curvature and give you a different value from a short upper bridge tangent.

Range

What counts as a small or large nasofrontal angle?

Clinical and aesthetic literature often places adult values broadly through the 120s and 130s. That is a descriptive zone, not a universal ideal. Smaller values create a stronger brow to nose break. Larger values flatten that transition.

A CT study of Arabian adults reported mean values of 125.3 degrees in men and 135.6 degrees in women (PMID 28377544). A separate quantitative study compared idealized and normative nasal dorsal values. Preferred and observed distributions are not the same question (PMID 27494590).

Do not turn one population mean into an attractiveness target. Forehead slope, radix height, bridge projection, sex cues, and ancestry all change what the same number looks like on a face.

Age

Does the nasofrontal angle change with age?

Yes, modestly. Paired CT scans separated by about 12 years found mean increases of about 3.2 degrees in men and 2.5 degrees in women (PMID 38062715). The same study reported changes in profile length and bridge height.

That result describes adult aging of the nasal profile. It does not prove suture expansion. Bone, cartilage, skin, and soft tissue all contribute. Longitudinal data beat comparing different age groups. You still need careful anatomical interpretation.

Population

Why sex and ancestry affect the angle

Three dimensional anthropometry in Korean adults with normal occlusion documented population specific soft tissue profiles, not one global norm (PMID 17561054). Studies of African American nasal form also report distinct profile relationships that matter when you analyse or plan treatment (PMID 7741429).

Sex differences partly reflect brow projection, forehead slope, radix height, and dorsal projection. The angle is a composite cue. Normalize it within the relevant population. Read it with the rest of the profile.

Capture

The errors that can move the angle

Head pitch changes forehead and bridge tangents. A three quarter image hides the deepest root point. A close lens enlarges the nose relative to the forehead. Brow tension moves the soft tissue glabella. Each error can shift the angle without any anatomical change.

Use a level side profile from a reasonable distance with moderate optical zoom. Keep the brow relaxed and include the full forehead, bridge, and tip. Repeat the same tracing rule every time.
