The nasomental angle is one number for how your nose and chin sit together. You trace it from nasion to the nasal tip, then from the tip to soft tissue pogonion. The tip is the vertex. A smaller angle usually means the nose projects more strongly relative to the chin. A larger angle can mean less nasal projection, more chin projection, or both. The number cannot tell you which structure caused it. Read it with separate nose and chin measurements.

How is the nasomental angle measured?
Use a true side profile. Mark nasion, pronasale, and soft tissue pogonion. Draw a line from nasion to pronasale and a second from pronasale to pogonion. The internal angle at pronasale is the nasomental angle.
Some analyses substitute different nasal root or chin points. That changes the construction. Always report the three landmarks. The name alone is not enough.
Measure nasal projection and chin projection beside it. If the nasomental angle changes, those component values show you whether the nose, the chin, or capture geometry moved.

What a small or large angle means
A smaller angle often means your nose projects more strongly relative to the chin. A larger angle often means a stronger chin, weaker nasal projection, or a flatter combined diagonal. Geometry, not a diagnosis.
Lateral cephalometric research shows that nasal shape correlates with the surrounding craniofacial pattern rather than varying independently (PMID 33809695). Read the angle beside maxillary position, mandibular position, and facial convexity.
Why chin preference changes the idealized angle
An image manipulation study found that preferred chin projection changed with sex and frontal facial type (PMID 28337420). You cannot pick one nasomental target for every face. The chin endpoint itself carries context-dependent preferences.
Nose shape, lip position, forehead slope, and lower face height also change the read. Ask whether the profile components balance. Do not ask whether every person hits one number.
Why nose and chin are often planned together
Long term follow up of combined rhinoplasty and genioplasty treats the nose and chin as a coupled profile problem rather than two isolated features (PMID 23493992). Change either endpoint and you change the nasomental relationship.
Those papers show anatomy can change the angle. They do not turn one group's postoperative target into a universal population norm.
Perspective error is especially severe here
Your nose sits closer to the camera than your chin. Short camera distance makes the nose look larger and the angle smaller. Small head rotation can hide the true tip and shift pogonion.
Use a long camera distance with moderate optical zoom, exact side pose, level head, relaxed lips, and the full forehead to chin profile. Compare only matched setups.
